Penicillin G benzathine is a bactericidal beta-lactam antibiotic that yields penicillin G through hydrolysis. Penicillin G inhibits the biosynthesis of cell-wall peptidoglycan, osmotically destabilizing the cell wall, in penicillin-susceptible organisms during active bacterial replication. Penicillin is inactive against penicillinase-producing bacteria or other organisms that are beta-lactam resistant due to alterations in penicillin-binding proteins

Deep IM only.
Rheumatic fever, recurrent; prophylaxis:
Adults, 1.2 MU q3-4wk.
Children ≤ 27 kg, 0.6 MU IM q3-4wk.
Children > 27 kg, 1.2 MU IM q3-4wk.
Syphilis:
Congenital: neonates > 1200 g, 0.05 MU/kg once.
Primary, secondary, early latent: adult, 2.4 MU IM once; children 0.05 MU/kg IM once (up to 2.4 MU/dose).
Late latent or latent syphilis of unknown duration: adults, 2.4 MU qwk×3 wks.; children, 0.05 MU/kg IM (≤ 2.4 MU/dose) qwk×3 wks. (total max. 7.2 MU)
Neurosyphilis: adults, 2.4 MU IM qwk×3 wks. (total 7.2 MU)
Streptococcal pharyngitis:
Adult, 1.2 MU once.
Children ≤ 27 kg, 0.6 MU IM once.
Children > 27 kg, 1.2 MU IM once.
